PROCEDURES FOR SITE EVALUATION IN JESSAMINE COUNTY

Site Evaluation Procedures
Please complete the attached application including basic site plan showing:

  1. Specific address or location of site and directions to site if necessary.

  2. Site boundary lines and dimension of site.

  3. Location of any existing structures, easements, sewage systems, ponds, streams, roads, and drives. Also indicate any future proposed structures such as swimming pools, detached garages, ponds or any other significant structure.

  4. Propose a location of structure to be served by the system, and desired system location.

  5. Submit with application a copy of floor plans including each level of structure.

All lots must be mowed, boundary lines clearly marked, and building site flagged. The applicant must provide a backhoe and an operator. Backhoe pits will be necessary to perform the site evaluation.

All site evaluations are completed on a first come first served basis. The fee for a site evaluation is $150.00. We can only accept check or money order. Please make checks payable to the Jessamine County Health Department. Please call (859) 885-2310 to schedule your site evaluation.
